My mom in law is 57 yrs of age. She suffers from type 2 diabetes and she recently learned that she is in complete heart failure. Maybe two weeks ago when she went to the doctor, it was told to us that her heart was weak. It was not pumping blood through her body the way that it should. In the weeks prior she had become swollen every over her body and the cause was unknown. She was admitted for that and then released with her being told to stay off of her feet as much as possible. She was even given a motorized wheel seat for her to continue to be mobile without putting pressure of her limbs to walk. On last Monday she went to the hospital for them to put a catheter in through her neck for something that had to do with her heart but upon arrival she had far too much fluid in her body and surrounding her heart to perform the surgery so they had to drain the fluid. She was admitted and the draining process began. By Tuesday she had filled 7 bags or so of fluid. When they connected another bag for drainage, the bag was filled with some black substance and blood. they had to immediately take her to surgery and she was taken to ICU after wards. In a day or so after the surgery we learned that her kidneys have completely close down and she was in renal failure. Dialysis was not possible because her heart was far too weak. At first when she urinated she was putting out a tiny amount of black fluid like substance that was somewhat thick. Now she is urinating as normal but the doctors are still saying her kidneys are not filtering as they should. They would not recommend her as a candidate for dialysis and if she were to go into cardiac arrest they are pretty much asking us to recommend not resucitating her and to let her die. She is medicated every the time which is causing her to either hallucinate or she starts talking about things that happened years ago. She can't filter the morphine they are giving her properly and they keep giving it to her. She still recognizes who we are individually but she is not altogether there. Sometimes she makes sense, sometimes she doesn't. We do not want to just accept that there is nothing else that they can do. If she is still urinating, is it possible for dialysis to work for her to prolong her life or would it be in vain? and if dialysis was not an option then how long would she linger like this before she passes away? She still has some kidney function in the way of urinating, just not filtering. What other options do we have? It seems the doctors want to just write her off.

So B-Blockers inhibit the synthesis of cAMP which in turn decreases Ca++ influx into the cell and decreases contractility (negative inotropic action). However by decreasing the rate of conduction through the AV node + blocking B1 stimulation, the HR slows which increases diastolic filling times and thereby theoretically increases contractility d/t Starling's Law. So which effect is more powerful, and is the net yield and increase or decrease in SV/SVI? I would wager that the SV/SVI actually increases to compensate for the decreasing rate to maintain cardiac output. Yeah? This is similar to how Dobutamine has a B2 action but also has an A1 action, leading to a net result of a slightly decreased SVR, but not as dramatically as previously thought because of it's mild A1 action.

I am a 21 year old caucasian male. I am overweight, however up until a few years ago I was a multi sport athlete. I used to be a smoker, but quit icy turkey 7 months ago. I had a cardiac workup done a year and a half ago which involved an echo, a stress echo, and a 24-hour holter, and every three (according to the cardiologist) came back perfectly healthy. I have severe anxiety and have for quite a while, and it is every centered on an obsessive fear of heart attacks. No fewer than 10 different doctors have told me I have a healthy heart and should not be worried, but I still feel symptoms. Occasional severe left elbow pain, tingling in my left ring/middle finger, occasional shortness of breath. I know that these are every possible heart symptoms, so is it my anxiety giving my psychosomatic symptoms, or should I be worried?

My dad owes over 20,000 dollars in child support. I am now 20 years old and my brother is still underage. We need a new place to move and my mom makes around 1,200 a month, so my dad offered my mom a deal. His deal is to remove child support off of him and he'll help every month with the rent.
